Project School Manager Request for Service

This form will be emailed to the Financial Services Help Desk and the Requestor Email for your records. A meeting with the Project School Manager will then be scheduled via Outlook to confirm the booking.

Terms & Conditions:

  1. Project School Manager fees are paid by the requesting Business Unit with a minimum booking period of one week.
  2. Bookings will be processed on a first come first serve basis
  3. Bookings may only be authorized by the head of school and by selecting the head of school in the requestor field below you confirm their authorisation for this booking
  4. Schools will not be charged for Sick leave
  5. Cancelled bookings incur a cancellation fee of 50% of the total fee unless those booking dates are subsequently filled.
  6. Project School Manager will send email notification of hours worked less sick leave weekly to Lorraine Crew (Financial Services) for initiating payment request by electronic T Form.
  7. The Project School Manager is to be provided with a suitable work location, phone access, computer and network access.
